Provided of course that it doesn't skip when playing, is the sound reproduced the same as it would be if the CD were perfectly clean? Or is there any kind of small noise or distortion caused by a little dirtiness?

As long as you can read the bits, or if you can't read all of the bits then as long as you can error correct what you can't read, then no, it won't make a difference.

If there is enough grunge to obscure enough bits that you can't error correct what you can't read, then yes, it will make a difference.
